Kaizen No-Michi New Student Application Form

If you are a new student looking to join our Karate School, you will be given this form to complete before you train.
If you wish to print off this form and complete before you arrive at the Dojo, it would be helpfull on your induction.
This form does not hold you to any kind of contract. It is required for our student data base as well as insurance, safeguarding and child protection.
If you are already a member of our Kaizen No-Michi and you have any change of details, please complete this form and hand it to the club instructors.

JKS Dan Grading Application Form

This form must be completed at least 2 weeks before your grading.
Members will be eligible to grade providing that they have a current JKS license and have met all the minimum requirements as set out on the syllabus page. They must also ensure that any additional club requirements are met and permission is granted from the Chief Instructors of Kaizen No-Michi.

Please use this check list to ensure you have completed your application form and attached the correct requirements. Failure to produce incomplete forms may result in being disallowed to grade.

JKS Online Licensing

All JKS license applications, new and renewals now need to be actioned through the new on-line system via the JKS England & Wales website.

If you are managing the payments on behalf of another student, you can still use the system.
There are tutorial videos on the website to guide you through the application process.
On the new system, students don’t send any forms or stamped addressed envelopes. This will now be done by and from the JKS office making things simple and straight forward for the member, therefore the postage and packaging will be added on top of the price for a new license application. So from 1st August new applications processed via the website will be £24.50.

It is expected that all members of Kaizen No-Michi must be individually licensed members of the JKS, therefore it is the responsibility of all students to make sure that they have a current JKS License/membership. Failure to do so will have potential serious implications, particular in relation to insurance and safeguarding.
